Scheduling Appointments

To schedule an appointment, please call our office Monday-Thursday between 8:30 am and 5:00 pm and Friday between 8:30 am to 12:00 pm. Our appointment staff may ask you about the nature of your visit to book the most appropriate appointment for you. We would like to schedule our OB patient’s first visit for 8-10 weeks. So please call to schedule at your earliest convenience.

Messages

We strive to return patient calls as soon as possible. Non-urgent calls and messages will be returned within 24 hours. Please call our office directly for any urgent issues and go to the nearest emergency room or call 911 in the event of any life-threatening emergency.  If you need an appointment, prescription refill or test results, please call during regular business hours.

Guest Policy

No children or guests are allowed in exam rooms except:

  • Newborns (8 weeks and younger) in carriers
  • One adult guest for the first new OB visit & OB ultrasounds only (Guest is allowed in our lobby only if seating is available.)

Canceling Appointments

If you need to cancel or reschedule your appointment, please call us as soon as possible. This will allow another patient to use your appointment time. We request cancellation 24 hours prior to your appointment.

Late Arrival

Please make every effort to be on time for your appointment. We respect the time of our patients, and our providers strive to stay on schedule so that you don’t have to wait long. If you arrive late for your appointment, we will try to fit you in as best we can. However, you may be asked to wait or on occasion, be asked to reschedule your appointment. We will charge $50.00 for missed appointments, or appointments canceled or rescheduled less than 24 hours after your appointment. By failing to cancel or reschedule your appointment three or more times we reserve the right to dismiss you from our practice.  

Emergencies

For our patients who are experiencing urgent medical problems, same-day appointments are usually available. For medical emergencies outside of office hours, a health care provider is always on call and can be reached by calling our office. Please do not call the emergency line if you do not have an urgent medical problem and can wait until normal business hours. We do not refill prescriptions or birth control pills via the emergency line. If you are experiencing a serious or life-threatening emergency, dial 911.

Registration

At the time of registration, and periodically thereafter, you will be asked to complete a registration package. This will help our personnel keep insurance information and demographics accurate.

Payment

Payment is due prior to services rendered! We accept cash, checks, and major credit cards.  You are financially responsible for services received. We will file your insurance claim, as a courtesy to you.

  • If a personal check is returned for insufficient funds or non-payment, there will be a fee of $50.00, per check.
  • All over-due patient balances will be sent to collections and charged a $40 collection fee in addition to the account balance.

Forms Completion

There is a form fee of $25.00; the fee must be paid prior to the physician completing the forms. It cannot be billed to your insurance company.

Medical Record Request

You are entitled to copies of your medical records, whether for yourself or another medical provider. You will need to sign a medical records release authorization (click here), which you may fax, mail, or drop off to our office. Our office will then provide a copy of your records in a timely manner for a nominal fee of up to $15.00 to cover costs. The following charges apply to copying records requested for Personal use, attorneys, insurance purposes, disability determination and various other reasons. $1 for each page, not more than 50 cents; $.50 for each page more than 25 pages; search fee of $5 and the actual costs of mailing the medical records. We will respond to all requests within 30 days.

Prescriptions

Prescription refills are processed only during our normal office hours. If you would like to order your prescription through a mail order pharmacy, you can obtain a mail order form from your insurance provider’s website. Mail or fax the completed form, along with the written prescription from your practitioner directly to your mail order pharmacy.
